A regional owner/operator of skilled nursing facilities throughout Illinois and Missouri engaged Blueprint to advise and oversee the confidential sale of its geographic outlier skilled nursing facilities in western Illinois. Located in Monmouth and Mascoutah, Illinois, the facilities totaled 135 skilled nursing beds, representing the opportunity for an incoming provider to capitalize on expansion into underserved Illinois submarkets.
Showcasing upside potential largely through census growth and operational efficiencies, Blueprint's process resulted in an acquisition proposal from an East Coast-based regional owner/operator experiencing recent rapid growth in Illinois.

Despite the eventual acquirer deciding not to pursue the acquisition upon initial approach, Blueprint’s persistent follow-up to showcase various paths to value creation resulted in renewed interest from the eventual acquirer. Given the very small buyer universe for this deal, Blueprint’s ability to keep the right buyer at the table was critical to achieving a successful outcome.

Blueprint worked hand-in-hand with the seller and incoming buyer to ensure timely and direct communication throughout the transaction process, culminating with the successful closing to start Q2 2025.
