Blueprint successfully advised on the sale of a five-community skilled nursing portfolio totaling approximately 420 beds located in Northeast OH. The portfolio represented a rare opportunity to acquire meaningful scale through a geographically concentrated group of communities with strong physical plants, established operating histories, and the ability to create a significant regional presence.
At the time of sale, three of the communities were cash-flowing, while the remaining two were operating near breakeven, providing a balanced portfolio profile with a combination of current performance and future upside opportunities. The communities benefited from well-maintained facilities which featured all private resident rooms, established market positions, and attractive operating fundamentals, generating strong interest from experienced skilled nursing operators and investors seeking growth opportunities in Ohio.

Leveraging Blueprint’s extensive relationships throughout the skilled nursing sector, the team executed a targeted marketing process that generated eight competitive offers from highly qualified groups. The level of interest demonstrated continued demand for well-positioned skilled nursing assets offering scale, operational opportunity, and a strong presence within a desirable Midwest healthcare market.

Following a competitive process, the portfolio was acquired by a seasoned national owner/DD for operator entering Ohio, with whom Blueprint has a proven track record of successful transactions. The buyer’s operational expertise, resources, and commitment to long-term investment positioned the communities for continued success under new ownership.
