Blueprint was engaged by its longstanding client, an Ohio-based owner/operator, to execute the strategic marketing and sale process and facilitate the exit from three skilled nursing and long-term care facilities spread across Ohio with approximately 280 total licensed beds.
The portfolio consisted of three facilities in secondary and tertiary markets, constructed between 1971 and 1985, and ranged in licensed capacity from 63 to 130 licensed beds. Blueprint positioned the value-add offering as an opportunity to acquire the portfolio and assume attractive, low fixed-rate HUD mortgage debt with over 20 years of remaining term.
Blueprint confidentially targeted regional providers and private investors, ultimately selecting a new entrant to the state eager to grow its regional Midwest footprint. Closing timing was critical, so to preserve facility quality points and reimbursement rates while actively working through the HUD loan assumption processes, the portfolio closing was bifurcated between the start of Q3 and Q4 2026.
