Blueprint was tasked by a public REIT and its longstanding operating partner to execute the strategic sale of three skilled nursing and long-term care facilities in Ohio with nearly 250 total licensed beds.
The portfolio consisted of two facilities in the Cleveland metro area and one in New Philadelphia located in southeast Ohio. The subject facilities were constructed between 1982 and 2006 and ranged in size from 48 to 149 licensed beds.

Blueprint confidentially targeted regional providers and mostly private investors, ultimately selecting an in-state provider eager to grow its footprint with added scale. To preserve the facilities’ quality points and reimbursement rates, Blueprint coordinated amongst all parties, collectively working expeditiously and collaboratively, to close the transaction on June 30th, approximately 90 days following offer acceptance.
