Blueprint was engaged by a private investment firm to facilitate the sale of Community Care Center and The Terrace; both skilled nursing facilities located in the Chicagoland market. The seller elected to monetize the assets and realize investment returns as the investment horizon was nearing an end and shift focus to new investment opportunities more closely aligned with their strategic objectives.
Located in Chicago proper and Waukegan, Illinois, respectively, Community Care Center was built in 1970 and is licensed for 204 skilled nursing beds. The Terrace, built in 1960, is licensed for 115 skilled nursing beds. At the onset of the marketing process, a nearby south-side 200-bed skilled nursing facility shut down operations, and as a result, Community Care Center received an influx of additional residents, which positively impacted operations and financial performance beginning in Q3 2025. This upward trend provided an excellent opportunity for the new owner/operator to inherit a well-occupied and cash-flowing operation.
Blueprint positioned the offering as a unique opportunity to acquire one of the largest skilled nursing facilities in the city of Chicago along with filling the geographic gap of a consistent and relatively well-occupied skilled nursing facility in the northern Chicago suburbs.
Ultimately, Blueprint’s targeted marketing process resulted in seven competitive acquisition proposals, and one of the largest Illinois private investment firms was selected as the incoming owner. Working hand in hand with the seller and outgoing operator, Blueprint and the seller’s perseverance throughout the process culminated in the successful closing, achieving a sale price of $20,900,000, or approximately $66,000 per licensed bed.
