A family-owned owner/operator engaged Blueprint to market a 200-bed assisted living community, licensed under New York State’s Assisted Living Program (ALP) in Medford, on Long Island. Opened in 2008, the community is one of the largest Medicaid-assisted living providers in Suffolk County and has stayed above 96% occupancy since 2023.
ALP licensure gives access to state-funded Medicaid reimbursement, and New York has issued very few new beds on Long Island. Blueprint built the process around that scarcity, and around the fact that an ALP community of this size rarely comes to market. Investor demand far outstripped the supply of ALP beds.
Blueprint launched the offering in February and drew a deep pool of bids from experienced New York operators and private capital. The team then ran a tightly managed competitive process among three finalists, with each round pushing pricing higher. A buyer was selected in May at a top-of-market price.
To get to closing, Blueprint’s Capital Markets team also helped arrange bridge financing for the buyer. The deal team expertly navigated the New York Department of Health’s complex change-of-ownership approval process, and the deal closed five months after the buyer was selected. The outcome underscores the strength of Blueprint’s integrated approach, bringing investment sales and capital markets together under one roof.
