Blueprint was engaged by a regional skilled nursing owner and operator to facilitate the sale of Monument Rehabilitation and Care Center, a skilled nursing and assisted living facility in western Nebraska. The seller decided to exit due to the facility’s geographic isolation while refocusing its operational efforts on ownership’s facilities within a different and more condensed regional footprint.
Located in Scottsbluff, Nebraska, the largest city in Nebraska’s panhandle, this 1960s vintage facility is licensed for 160 skilled nursing beds and 23 assisted living units. Situated across from the only regional hospital within 30 miles, Monument encountered typical operational challenges following the pandemic; however, the outgoing operator significantly improved operations during the sale process, primarily led by increased census and quality payor mix. This recent upward trend provided an excellent opportunity for the new owner/operator to inherit a well occupied and cash-flowing operation.

Blueprint positioned the offering as a unique opportunity to acquire a positively trending facility at an attractive price-per-bed basis. Ultimately, a newer entrant to skilled nursing ownership, with an existing relationship with the outgoing operator was selected as the buyer and incoming operator. Blueprint and the seller’s perseverance throughout the process culminated in the long-awaited successful closing, achieving a sale price of $8,000,000.
